My name is Tara Phillips

Hi, I am a mother and grandmother. My gifts and talents are in the area of teaching. I have a Master's degree in Educational Psychology and a Master's degree in Social Work. I am also a Certified Deliverance Minister. I love to teach others about the love of Christ and how to connect with Him. Let me help you develop a Life Plan that incorporates your needs and desires that align with the word of God.

Being a follower of Christ, can be really difficult when you can't see him to follow him. I love connecting people with Christ. I will help you see how your everyday life can be impacted directly by the word of God. Let me help you "make it make sense". Sometimes we don't know how to get where we 'think" we want to go. Sometimes we don't even know where to start. Let me help you develop a life plan so you can run on the path that Christ has created for you. I specialize in helping people identify the roadblocks and stumbling blocks that are keeping them feeling disconnected from God. Once you are able to clear your path, your communication with God and understanding of His words becomes clearer.

You are not here by coincidence. God has equipped me to assist you. Go to my link and schedule your initial 2-hour virtual session with me. You will complete 2 forms and return them to me. This will give me valuable information prior to our session. A future 1-hour follow-up is also included, which we will schedule at the end of your first session.

4. You will also receive an email to complete the Christian Life Planning Questionnaire. I will review this infomation before we meet.

5. Join the Zoom link on the day of your scheduled 2-hour session. Be ready to talk about you. In the interim, I will be praying for you and asking for God's guidance in this process.

CHRISTIAN LIFE COACHING AGREEMENT:

The following represents the contract between Parks Concepts LLC and the Client. The services contracted for shall be those of life coaching. Services are conducted via video sessions. Christian Life Coaching is a partnership that focuses on helping the Client identify positive ways to change their lives through Jesus Christ. Coaching will focus on goals that are beneficial for the Client. These life goals may include spiritual, physical, emotional, mental, and social aspects of the Client’s life. Making positive life changes is a process.

As a Client of Parks Concepts LLC, you have the following rights:

To be entitled to respect and dignity in an environment that affords security and privacy;
To receive services that are protected under the laws of confidentiality. All conversations between the Coach and Client will be kept confidential. The only exception would be if there was an imminent threat of serious harm to the Client or others.
To be free from psychological abuse, including humiliating, threatening, and exploitive action on the part of program employees.
To receive services regardless of race, sex, national origin, creed, physical or mental handicap
To be informed that coaching is NOT a substitute for counseling, psychotherapy or mental health and/or substance use treatment. You will not receive diagnosis, treatment, or therapy.
To receive a 2-hour initial individual meeting by video to develop a life plan for the next year. To receive a follow-up 1-hour individual meeting by video within 6-12 months after the initial session.
To receive clear understanding of fees and services included.
The total fee is $400 due in advance of initial session; this fee includes the initial 2-hour session and the additional 1-hour session within 6-12 months following the initial session.
To receive a written plan from the Coach after the initial session.
To receive guidance, support, and feedback on two occasions, initial session and follow-up session
This agreement is terminated after the follow-up session is completed or after 12 months, whichever comes first.

CLIENT’S RESPONSIBILITIES

To agree to be as honest as possible with the Life Coach in order to develop a comprehensive and viable life plan.
To work in collaboration with the Life Coach to develop a life plan.
To agree to follow the plan and modify it as needed throughout the year. The Life Coach cannot change the Client. The Client takes responsibility for needed life changes. Change is a process.
To use the plan and safely make needed life changes and adjustments in alignment with Client’s goals.
To attend follow-up session within 6-12 months after the initial session.
To be informed that appointments may be rescheduled at the discretion of the Life Coach.
A 24-hour notice is required for cancellations or rescheduling.
Every effort will be made to reschedule the appointment in a timely manner.
If appointment is cancelled and NOT rescheduled, a 80% refund will be provided to offset processing fees and inconvenience.
Either the Life Coach or Client may terminate the agreement immediately upon notice. Notwithstanding the foregoing, Client shall not be relieved of any obligations to pay fees due to Parks Concepts LLC solely because of the termination of the agreement.
Limitation of Liability-in no event shall Parks Concepts LLC be liable to the Client for any special, punitive, incidental, indirect, or consequential damages of any kind in connection with this agreement.
This contract constitutes the entire agreement between the Client and the Life Coach. The Life Coach shall not be held responsible and shall remain blameless for any adverse or other consequences, including but not limited to financial, personal, employment or other losses, arising out of decisions or actions the Client may make as a result of the Services provided by the Life Coach.
